| How to Fill In the Registration Form |

Applicant’s Personal Information |
a. |
Select the Status of the applicant (Day/Boarder) |
b. |
The Registration No will be automatically generated after successful submission of the Registration form. |

Registration Date will be date on which the registration is done. It cannot be changed. |
d. |
Gender: There will be a Radio button with values Male / Female. User must select any one. |
e. |
Select applicant's Date of Birth in DD / MON / YYYY format from the respective dropdown list |
f. |
Select Class from the dropdown list |
g. |
Enter Nationality, Mother Tongue and Religion |
h. |
Select Category and Blood group from the dropdown list |
i. |
If the applicant is an NRI, click on the Yes button |
j. |
Fill in Communication Address and Pin code |
k. |
Fill in the 10 digit mobile number. [+ Country Code can be attached with the mobile number] |

Parent’s Personal Information |
a. |
Enter the Name / Academic Qualification / Occupation of the parents |
b. |
Enter the Occupation, Organization, Designation, Office Address, Phone no/Mobile no and E-mail. |

Click on the required check box of the area in which your child can contribute to the school. |

Fill up the box if the child has any special interests |

| Important information to be filled up for short listed candidates |

Fill up the box if applicant's parent / parents employed in DPS Numaligarh |

Fill up the sibling details of the applicant (if any) if studying in DPS Numaligarh |

After filling up all the required details click on the Submit button. Once submitted successfully, the applicant will get a confirmation page informing about the successful submission along with the REGISTRATION NUMBER.
After successful submission, the form can be viewed / downloaded / printed.
Online payment is compulsory to complete the application submission. Non-payment of Registration Fee confirms that the application form is not complete. |
